APL’s D-4 Group Dental Insurance may be the perfect answer to your group’s dental health needs. D-4 offers a Scheduled Benefit Plan,
which means that each of the over 200 covered dental procedures is listed with a corresponding dollar benefit amount. Each covered
procedure is listed by ADA Code, which is the code that dentists use to bill. Depending on which plan you select, APL reimburses
for the listed benefit amount. There is no guessing! You know exactly how much the D-4 Scheduled Dental Plan will pay.
D-4 Dental is only available for sale through the convenience of payroll deduction.
